Our primary aim as dentists is to preserve teeth. There are times, however, when preserving a tooth is no longer worth the effort and we must recommend removing it. Fortunately, extracted teeth can be replaced with a functional and attractive restoration.

Today's top tooth-replacement option is the dental implant. Composed of a titanium metal post imbedded into the jawbone, a single dental implant can replace an individual tooth or a series of implants can support other restorations for multiple teeth. Besides being incredibly life-like, dental implants are highly durable and can last for decades.

But dental implants aren't an optimal choice for everyone. Their cost often matches their status as the premier tooth replacement method. And because they require a minimum amount of bone for proper implantation, they're not always feasible for patients with extensive bone loss.

But even if dental implants aren't right for you, and you want a fixed restoration rather than dentures, you still have options. What's more, they've been around for decades!

One is a bonded crown, which works particularly well for a tooth excessively damaged by decay, excessive wear or fractures. After removing all of the damaged portions and shaping the remaining tooth, we cement a life-like crown, custom created for that particular tooth, over the remaining structure.

Besides improving appearance, a crown also protects the tooth and restores its function. One thing to remember, though, is although the crown itself is impervious to disease, the remainder of the natural tooth isn't. It's important then to brush and floss around crowned teeth like any other tooth and see a dentist regularly for cleanings.

Dental bridges are a fixed solution for extracted teeth. It's composed of prosthetic teeth to replace those missing bonded together with supporting crowns on both ends. These crowned teeth are known as abutments, and, depending on how many teeth are being replaced, we may need to increase the number of abutments to support the bridge.

Although durable, crowns or bridges typically don't match the longevity of an implant. And, implants don't require the permanent alteration of support teeth as is necessary with a bridge. But when the choice of implants isn't on the table, these traditional restorations can be an effective dental solution.

Dental plaque, that gritty bacterial film coating your teeth, is the top cause for tooth decay and periodontal (gum) disease. You can see and feel a lot of it—but not all of it. Some deposits can lodge snugly between your teeth, and can cause dental disease just as much as what's out in the open.

The problem with between-teeth plaque is that even a solid brushing habit might not effectively remove it. That's why you flossing should also be part of your daily oral hygiene.

If the thought of flossing, however, causes you to let out an audible sigh, we understand. Flossing typically engenders less enthusiasm than brushing, mainly because many find flossing time consuming and difficult to do.

If traditional flossing isn't your bag, we may have a reasonable alternative. Oral irrigation is a hygiene method for removing plaque between teeth using a pressurized water spray. You direct the water spray between your teeth using a handheld wand (which somewhat resembles a power toothbrush) and small hose attached to a countertop pump appliance.

A mainstay in dental offices, oral irrigators (or water flossers) have been available for home use since the 1960s. They're ideal for people who have problems with manual dexterity or who may not want to contend with flossing thread. They also make it easier for patients wearing braces to clean between their teeth, a monumental task using regular floss.

As to effectiveness, oral irrigation appears to match that of regular flossing, especially for orthodontic patients. Clinical studies in the early 2000s compared patients with braces using oral irrigation with those who were brushing only. Those using irrigation were able to remove five times as much plaque as the other group.

There are a number of comparable oral irrigation brands on the market from which to choose, and your dentist can advise you on features to look for when purchasing one. Just be sure you're using some method, oral irrigation or traditional flossing, to remove disease-causing plaque from between your teeth—either will go a long way in keeping your teeth and gums healthy.

Braces are so common that we often view them as "ho-hum." But there are aspects about braces that make them remarkable. For one, the fact that we can move teeth at all is a wonder of nature.

We normally experience our teeth as firmly set in the jaw, which can easily lead to assuming they're permanently fixed to the bone. They're not. Teeth are actually held in place by a fibrous gum tissue called the periodontal ligament that lies between them and the jawbone. The ligament anchors to both with tiny fibers, which on the tooth side affixes within a thin substance called cementum deposited on the tooth root.

As we said, we don't normally notice teeth moving. But the periodontal ligament does allow movement on a miniscule scale as a response to normal pressures that accompany biting and chewing. Although we're unaware of it, this movement takes place as the bone and cementum ahead of the direction of movement begin to dissolve. Simultaneously, new bone and cementum develops on the other side of the tooth to stabilize the movement.

Orthodontic treatment takes advantage of this natural process. The anchored wires of braces through attached brackets place pressure on the teeth in the intended direction for tooth movement. The natural mechanism described earlier does the rest. Over time, orthodontists have developed an amazing amount of precision working within this mechanism.

Another aspect about braces and other methods we may take for granted is our motive for even trying to move teeth in the first place. It may seem we're only realigning teeth to produce a more attractive smile—which they can do and why we often refer to braces as the "original smile makeover." But there's an even greater desire—straightening teeth can improve dental health.

Poor bites in turn cause other problems. Misaligned teeth are more difficult to keep clean of bacterial plaque, which increases the risk of disease. A poor bite can also accelerate teeth wear and contribute to gum problems like recession. We can eliminate or minimize these problems through bite correction.

Whatever your age, braces or other means can vastly benefit your health and your appearance. They may not always seem so, but braces are one of the true wonders of dental care.

During the COVID-19 quarantines, stir-crazy celebrities have been creating some “unique” home videos—like Madonna singing about fried fish to the tune of “Vogue” in her bathroom or Cardi B busting through a human-sized Jenga tower. But an entertaining Instagram video from Kevin Bacon also came with a handy culinary tip: The just-awakened film and TV actor showed fans his morning technique for cutting a mango to avoid the stringy pulp that gets between your teeth. After cutting a mango in half, he scored it lengthwise and crosswise to create squares and then turned the mango inside out for easy eating.

With his mango-slicing video garnering over a quarter-million views, the City on a Hill star may have touched a nerve—the near universal annoyance we all have with food stuck between our teeth. Trapped food particles aren't only annoying, they can also contribute to a bacterial film called dental plaque that's the top cause for tooth decay and gum disease.

Unfortunately, it's nearly impossible to avoid stuck food if you love things like popcorn, poppy-seed muffins or barbecue ribs. It's helpful then to have a few go-to ways for removing food caught between teeth. First, though, let's talk about what NOT to use to loosen a piece of stuck food.

A recent survey of more than 1,000 adults found that when removing something caught between our teeth, we humans are a creative lot. The makeshift tools that survey respondents said they've used in a pinch included twigs, safety pins, screwdrivers and nails (both the hammer and finger/toe variety). Although clever, many such items are both unsanitary and harmful to your gums and tooth enamel, especially if they're metallic or abrasive.

If you want a safe way to remove unwanted food debris, try these methods instead:

Brush your teeth: The gentle abrasives in toothpaste plus the mechanical action of brushing can help dislodge trapped food.

Use dental floss: A little bit of dental floss usually does the trick to remove wedged-in food—and it's easy to carry a small floss container or a floss pick on you for emergencies.

Try a toothpick. A toothpick is also an appropriate food-removing tool, according the American Dental Association, as long as it is rounded and made of wood.

See your dentist. We have the tools to safely and effectively remove trapped food debris that you haven't been able to dislodge by other means—so before you get desperate, give us a call.

You can also minimize plaque buildup from food particles between teeth by both brushing and flossing every day. And for optimally clean teeth, be sure you have regular dental office cleanings at least twice a year.

Thanks to Kevin Bacon's little trick, you can have your “non-stringy” mango and eat it too. Still, you can't always avoid food getting wedged between your teeth, so be prepared.

Half of adults over age 30, and an astounding 70% over 65, have had some form of periodontal (gum) disease. Unchecked, a bacterial gum infection can spread into the supporting bone and destroy attachments between the teeth and gums. Because of its rapidity and aggressiveness, gum disease is the number one cause of tooth loss among adults.

But there may be even more harm caused by gum disease beyond losing teeth: There's growing evidence gum disease may worsen other diseases like diabetes, heart disease or rheumatoid arthritis. Patients with gum disease are also more likely to suffer from one or more of these systemic conditions.

The link between gum disease and these other diseases appears to be inflammation. When tissue becomes injured or diseased, swelling (inflammation) occurs to isolate these tissues from the rest of the body. Under normal circumstances, this is a critical defense mechanism to protect the body overall.

But this response is a temporary measure—if it becomes chronic, it can actually damage the tissues it's trying to protect. This often happens with gum disease as inflammation can't overcome the gum infection, and both sides settle into a kind of trench warfare. The same story plays out with other diseases with an inflammatory response. And if the body is waging war with a gum infection, it can worsen these other conditions.

It's important then to take care of your gums and the rest of the body to minimize chronic inflammation. You can help prevent a gum infection by brushing and flossing every day and getting your teeth cleaned professionally at least every six months. You should also see your dentist if you notice swollen, reddened or bleeding gums, often the first signs of gum disease.

It can also benefit your gums if you're addressing other inflammatory issues in your body. Besides regular medical care, you can reduce your risk for other systemic diseases by eating a healthy diet, keeping your weight at an optimum level and avoiding smoking.

The individual parts of your body aren't isolated islands: Diseases that affect one can eventually affect all. By preventing or treating gum disease as early as possible, you'll also help reduce the effects of other systemic diseases.
